LMS Students must fill out RIPCOA Assumption of Risk Form once during Lincoln Middle School career
PHYSICAL EXAMINATIONS
Students registering for Athletics must have a current physical examination on file in the nurse’s office before the student is authorized to participate in practice/tryouts. To ensure clearance for participation, physicals should be completed and turned into the nurse no later than 1 week prior to the first day of practice/tryouts. All physical forms must be handed to (or emailed to) the school nurse.

Physicals are valid for 13 months from date of the last exam.
RIIL ASSUMPTION OF RISK FORM (Needed for LHS Students)
All student-athletes must bring (or email) a notarized assumption of risk form to the Athletic Director's Office (or mailbox in main office) prior to the 1st day of practice or tryouts.
* As of 8/6/2020 there is a new risk form required for all high school student-athletes. If you have submitted a form in previous years, you are still required to submit this new form. Please click below for the new form.
